COORDINATED

Care coordination is  organizing all patient activities and information among all health professionals   concerned with a patient's care. This coordination helps  achieve safer and more effective care.

SHOULD I FEEL GUILTY WHEN CONSIDERING A MEMORY CARE FACILITY FOR MY LOVED ONE?

Alzheimer's and other forms of dementia are frequently a progressive disease. Over time, a senior's memory loss increases which prevents the ability to live independently.  The level of care needed becomes too great for family and other caregivers to manage.  Once the memory loss progresses and becomes more advanced it becomes difficult to safely remain in a home setting.

Advanced dementia requires a safe, secure setting with care provided 24 hours a day, 7 days a week. In advanced dementia and memory loss people have an increasingly difficult time with activities of daily living (ADL’s).  These activities include dressing, bathing and using the toilet.  Many times additional cognitive issues worsen and create issues such as aggression, disorientation, and confusion.

The demands of caring for a loved one with dementia can become increasingly difficult for caregivers at home. Many times the caregivers health begins to deteriorate due to the stress and demands of caring for the family member with dementia.  A Memory care community offers the care and support needed to properly and effectively care for those with memory loss alleviating the stress and pressure on family and caregivers.

Some questions you may ask your self when considering an assisted living facility for a loved one:
  1. What are their current living arrangements and how do they compare to memory living?

  2. Do they need help with activities of daily living, such as bathing, dressing, or eating?

  3. Would they benefit from having access to on-site medical care and assistance?

  4. Would they prefer to be surrounded by people your own age, with frequently scheduled social events?
